Short :
Simply a form you can fill out that will give some information about your team, making it easier for newcomers and unknown to learn about your team, what the sensei's opinion is, theme song and what not?
Jounin's need to update it themselves, by updating it and then send the folder to a GM. Then it should be updated on the roster asap. It is that simple.
